One that has not happened to me recently but still deserves a mention:

When producers leave out important details about how a line should be said or a character should be voiced and either ask for redoes with no direction whatsoever (or horribly inadequate direction) or simply leave it as-is and let it just not sound right in context -- especially when the production is a radioplay where the actors do not have the visuals to clue them in to the missing details. (I've even seen this happen in professional dubs, everything from characters not playing off of each other to actors giving the wrong pronunciation for a word, e.g. "bow" as in a polite gesture vs. "bow" as in a device used to shoot arrows.)

Dang. Lol. I just did this recently. I always forget to say what kinda voice. But usually, I want them to use their regular voice. Usuallyyyyyyyyyyyyyyyyy. D: Still. It wouldn't hurt to tell them. I'm slowly remembering to do that >.>

This is a hilarious pet peeve, but I can't find a good "hug" or "embrace" sound effect to save my life. Not that it's important, but most of my "body contact" sound effects tend to be on the aggressive side.

What happened to peace and love in the world of SFX? XD

This. I have no idea how to portray a hug/embrace in a radioplay because of the lack of SFX for it. And putting "Omgz lets hug" in the script so listeners know they're hugging, would be really weird.

If nothing else, try a pat on a sweater sound effect. Otherwise, it's just music or dialogue.

I would look for some sort of clothing movement sfx. Maybe a bed sheet being moved or something.
