People who call me out for VAing. One incident comes to mind...
On Youtube:
"Hey xxxxx (can't reveal my name, sorry), what is this? Lol..." On one of my auditions.
And it's been half a year since that, and he STILL mentions it. GAH!

One of my pet peeves is hearing anyone say "casted" which is not a word:
http://www.merriam-webster.com/dictionary/casted
"Cast" is past, present and future.

Yes, I see 8 pages of search results for "casted" just in this thread alone.
But it's worth repeating, especially since there are members here still using it in their sig lines.
Another pet peeve of mine is people thinking that because they can do "funny voices" it automatically makes them a "voice actor." As YouTube has proven many times over, the voices are often amusing only to the ones performing them.
And I think I dislike this one most of all -- the most frequently asked question at conventions: "How do you get into voice acting?"
There's this thing called "The Internet" and another thing called "Google" and yea, verily also yet another thing called "Wikipedia" that will help to answer that question before you waste valuable panel Q&A time with a question that has been answered so very many times before. When I host VA panels at cons, I don't even allow that question to be asked. I have the panelists each discuss how they got started and each give their spin on advice for how to get started.
But what frequently happens is someone comes in late and asks the same dang question yet again. It makes you want to pull your hair out.
Related to this, I cringe every time a fan asks a question either with a goofy character voice or by imitating the character voice the panelist is known for. Seriously, don't take advantage of panel Q&A by thinking you're being clever or that this is somehow a chance for you to be discovered with an audition that nobody wants to hear. If you wish to show your appreciation to a voice actor by performing an imitation of their character(s), please do it during their autograph session. Don't make a public spectacle of yourself. Don't be "that kind" of fan.

When your voice cracks randomly in the midst of a monolouge. A very long monolouge.
GOD. FUCKING. DAMMIT.
This has always and will always bother me.
People that don't release their projects ! C:
Especially bothers me when you are in it . xD Most all of the things I ever get cast in never make it to the light of day.. So that has stopped me from auditioning for much anything anymore these days .. x3~
Things with serious potential which are ruined with a few bad casting choices. Here's hoping they go beyond what I expect.
Also, remake of game I like: "Wow! I could voice so-and-so, it'd be so fun!" Send in auditiony thing. "Hi, it's really good, but so-and-so's character has been rewritten to the point of needing a male voice actor." B-but, he's nine...

People that needlesly precast themselves in a project because they "like the character", regardless whether or not they could actually pull it off.

I just noticed that I was safe in my sigYes, I see 8 pages of search results for "casted" just in this thread alone.

And I didn't even know casted wasn't a word XD
