Oh go to the form and click "edit" (like the first one where u added the cast) then click "go advanced" and then there shuld be something saying "Title" on the top and u can just change that

Hey i forgot to mention this and i just noticed this but ur form says "【OPEN!】Fruits Basket Fandub Project Auditions!" You should probably change that "OPEN" to "Cast List Posted" or something like dat ;]
